NGR 6014: Management of Population Health and Illness Across the Continuum of Care for Nurse Leaders

This course focuses on the role of nurse leaders contributing to interdisciplinary teams for the management of chronic illness, population health, and care coordination across the continuum of care. Emphasis will be placed on health outcomes, patterns of health determinants, and the methods by which these are addressed. Topics will include chronic illness management, epidemiology, ethics, genetics and genomics, social determinants of health, public health care program planning, disaster/pandemic preparedness, strategic planning, data management, and outcomes evaluation.
